POSSIBILITIES FOR TREATMENT OF PATIENTS WITH PELVIC RING INJURIES

The authors present the experience of using minimally invasive techniques for osteosynthesis of acetabular and pelvic fractures in 84 patients in 2003–2016. The age of patients ranged from 17 to 70 years. The authors used cannulated screws and threaded wires for osteosynthesis. Indications for the use of minimally invasive osteosynthesis of pelvic fractures have been detected. Minimally invasive immobilization of acetabular fractures in the early stages is technically less complicated and traumatic; it provides the prevention of quiet hip disease and favorable conditions for fracture healing.

THE PROGNOSTIC MODEL OF PATIENTS WITH POLYTRAUMA. THE PILOT PROJECT

The predictive model for assessment of survival of patients with polytrauma was developed using the multivariant analysis technique (logistic regression). The level of enzymatic activity of cathepsin D (after 3 and 48 hours) is a biomarker (predictor) of survival for these patients. The study results make it possible to recommend the developed model for validation (with potential correction of the coefficients) and use in clinical practice.

CRITICAL FLAME BURN. CASE STUDY

Thermal injury remains one of the most common types of injuries and accounts for 30% of all types of injuries according to the World Health Organization. It is a serious medical and social problem with high mortality, significant rates of temporary labor losses and primary disability. The developed at the Main Military Clinical Hospital of the National Guard Forces Command of the Russian Federation local and surgical treatment technology is salvation in some case of critical burns with a shortage of donor sites. The article shows the example of successful treatment of a soldier with a critical burn in order to illustrate the resource of skin restoring in such patients and bringing them back to life.

The article deals with the assessment of clinical efficacy and safety of Prestance and Arifam in therapy of arterial hypertension and inter-visit variability of blood pressure in elderly patients. The authors note the positive effect of drugs on reaching the target level of blood pressure and decreasing inter-visit variability of blood pressure to levels that reduce the risk of death from cardiovascular complications and overall mortality. The drugs showed good tolerance and compliance with the therapy.

In recent years, many researchers have been paying attention to the increase in the proportion of systemic lupus erythematosus in the group of autoimmune diseases. This disease is characterized by a variety of clinical forms but rare cases of the disease in young men are of particular interest. The article shows the case of progression of discoid lupus erythematosus to systemic lupus erythematosus in the male of young age.

Road traffic accidents cause great social, material and demographic damage to the Russian economy and society. Inclusion of obstructive sleep apnea syndrome in the list of diseases that may be grounds for limiting driver’s licenses will reduce road mortality by 20%. This justifies the need for adjustments of the current legal acts on compulsory medical examination of drivers.
Health care and social protection of the population are the most urgent problems, especially in the context of the recently changed social, economic and political situation in the country. Return to work, socially useful activity of persons with temporary or permanent disability, disability prevention, improving the quality of life of work-related accidents victims and persons with occupational diseases is an important state task.
